Eligibility
The Community-Based Adult Services (CBAS) program provides essential healthcare and social services to eligible individuals who require support to maintain their independence and overall well-being. Eligibility is determined based on medical, cognitive, and functional needs.
To qualify, individuals must meet the following criteria:
1. Age and Medical Necessity
- Must be 18 years or older and require services due to a medical or chronic condition.
- Seniors aged 65 and older who require additional support to maintain their health and independence.
2. Functional Impairments
- Individuals must have functional limitations that make it difficult to perform daily living activities independently.
- Must require assistance with two or more Activities of Daily Living (ADLs) or Instrumental Activities of Daily Living (IADLs) due to medical, physical, or chronic conditions including:
- Ambulation
- Accessing Resources
- Hygiene
- Meal Preparation
- Medication Management
- Transportation
- Must be in need of assistance and/or supervision.
- Independent, Needs Supervision
- Dependent, Needs Assistance
3. Social Isolation
- Individuals who would benefit from socialization and structured activities to improve mental health and prevent isolation.
- Lives alone without family or a caregiver to provide sufficient and needed care or supervision.
- Lives with one or more related or unrelated individuals who are unwilling or unable to provide sufficient and needed care and supervision.
4. Risk of Institutionalization
- Individuals must be at risk of nursing home placement without access to CBAS services.
- The program serves as an alternative to long-term care by providing medical and therapeutic services in a community-based setting.
